Utility Clearance Forms and/or Construction/Engineering Contact

We have a major Land Development Project in the City of Allentown and submitted a package containing plans and utility clearnace forms to AT&T corporate in Texas but never received a reply. Who would be the appropriate contact in regards to a Utility Clearance? Thank you for your time.

Form D – 4181UC - Utility Relocation Clearance Report Instructions (three pages)

These forms are required by PennDOT in the formulation of the clearance of necessary utility relocations that may be involved with the above described project.

Please note, Form D – 4181 must be completed by each utility company even if the PA One Call states that the utility is not affected.

Please note Form D – 4181UC only needs to be completed by the utility company if the project will have impacts on their utility.
