Fan Pin

Our company is trying to create our account online and can't because we weren't given a FAN PIN. Also, we tried to speak to a representative on the phone and were told that we don't have authorization and something must be signed. Can someone assist me in sending a form so that our owner can sign it as well as provide the FAN PIN? Would greatly appreciate help as soon as possible because we are experiencing serious tech problems with one of your equipment and no way to talk to you about it until this gets resolved.
