Getting billed multiple times for disconnected account

we have circuit in Sandiego, and we have disconnected it two years back and we are still getting billed for that. we have reached our account manager alex   he said the disconnection was not done properly he once again initiated the process, but we are not getting proper response. we need to escalate this immediately even this month we received the invoice please let us know the process to resolve this we need refund for the amount that we paid and the account is bundled with other account we are confused on that how this can happen.
